Creating a YourHealthFile patient portal account will give you easy access to securely view your medical records online.  With access to the patient portal, you can: 

  • Pay your medical bills online
  • Review your personal account information
  • Review a detailed summary of your health record and the results of your last office visit
  • Review your medical records including medications, immunizations, allergies, problem list, lab results, office tests and more 
  • View upcoming appointments 
  • Communicate with the physicians and staff via a secure messaging system

If you share information from YourHealthFile account information with another individual, you acknowledge and accept responsibility for your decision to provide them access to potentially sensitive information.

If you learn or suspect that your username or password has been wrongfully used or disclosed, you should promptly notify us and immediately or follow the steps below to reset your password. To help ensure the security of your password or account, please sign out of your account at the end of each session.

What if I don’t know my username? From the Login Page at https://www.yourhealthfile.com, Click the link “Forgot Username or Password??” From the “Forgot Username or Password?” Page > Select the “Forgot Username”. Follow prompts to complete. If you are still having issues please contact care provider’s office

What if I forgot my password? From the Login Page, Click the link “Forgot Username or Password??” From the “Forgot Username or Password?” Page > Select the “Forgot Password”. Follow prompts to complete. If you are still having issues please contact care provider’s office.

What if I forgot my Username & Password? From the YourHealthFile Login Page, Click the link “Forgot Username or Password?” From the “Forgot Username or Password?” Page > Select the “Forgot Username & Password”. Follow prompts to complete. If you are still having issues please contact care provider’s office.

Why can’t I login? Please read the error messages carefully and complete the required information. If the issue continues, please contact your practice.

If you are a new user and have not yet created an account, please select the option at the bottom of the initial login screen for “First time user? Activate your account” You will then be guided through creating a login for the portal.
